Appointments can be scheduled over the phone. We offer to schedule your nest biannual cleaning during your office visit or our system can send you a text or email about 6 weeks before your appointment is due. We ask for at least a 24 hour advance notification should you need to cancel an appointment as it is difficult to fill the time without adequate notice. While we understand this is sometimes impossible to do, appointments cancelled without 24 hour notice may be subject to a fee.
On your first visit you will be asked to fill out paperwork including health history, insurance information, dental history, consent to treat, and privacy information. You can find all of the paperwork in the Patient Information section of our website. One of the pages is an authorization to release records. If you have been to the dentist within the past five years, please complete this form and submit it to your previous dentist. If it is easier, you can complete the form and email it to our office at least 48 hours prior to your appointment and our staff will follow up on your behalf. Providing past x-rays and treatment information is necessary for proper diagnosis and will help us greatly as we work with you on your dental health goals.
Fees for dental services are controlled to the extent possible. It continues to be part of our belief that quality dental care should be available to everyone. Before any work is started. we discuss a treatment plan with you and provide an estimate of charges. If the amount of dental work you require is significant, we will work with you to prioritize treatment. We want to be sure that you are aware of potential consequences associated with delaying treatment but understand the need to plan for balance charges. We also have payment options including credit cards and Care Credit.
Dental Insurance is an aid to help defray costs and a benefit offered by many employers. We do our best to maximize insurance benefits but we can not always predict what your insurance will pay, as every insurance plan is different. We will file a claim with your carrier after your visit as a courtesy to you.
